Seeking a senior Epic Healthy Planet Analyst with strong population health and value-based care experience.

\n

Required:

\n

\n
• Epic Healthy Planet Certification

\n

• Healthy Planet build and optimization experience

\n

• Registry build and maintenance

\n

• Reporting Workbench and dashboard development

\n

• Population health workflow analysis

\n

• Quality reporting experience (HEDIS, eCQMs)

\n

• Care management and care gap management experience

\n

\n

Highly Preferred:

\n

\n
• Compass Rose experience

\n

• Risk stratification and predictive analytics

\n

• Patient attribution frameworks

\n

• External data integration (claims, SDOH, HIE feeds)

\n

• Medicare Stars and value-based care programs

\n

• Clarity/Caboodle experience

\n

• Care management tools, care plans, assessments, and outreach workflows

\n

\n

Responsibilities:

\n

\n
• Configure and support Healthy Planet registries, risk models, care gap logic, and attribution workflows

\n

• Develop Reporting Workbench reports, dashboards, and analytics

\n

• Support HEDIS, Medicare Stars, and other quality programs

\n

• Build and optimize care management workflows and tools

\n

• Validate data between Epic, Clarity, Caboodle, and downstream reporting platforms

\n

• Troubleshoot population health reporting and data integrity issues

\n

• Partner with clinical, quality, and population health leadership teams

\n

• Support upgrades, testing, optimization, and end-user support

\n
