Job DescriptionSalary: Starting $24.04 hr.

***This job requires travel throughout Greene County and would be a mix of in office, remote work and in-person visits.***

As a Service and Support Administrator for Greene Co Board of DD, you'll get to:

  • Coordinate necessary assessments for purpose of developing the Individual Service Plan (ISP).
  • Develop, review, and revise Individual Service Plans (ISP) in accordance with person-centered planning principles, administrative rules and within required timelines.
  • Ensure that necessary services are immediately available on a twenty-four (24) hour basis where a situation develops that is urgent and in need of immediate attention. Participates in rotation of on-call duties.
  • Implement a continuous review process that is tailored to the individual and identified in the Individual Service Plan (ISP).
  • Provide on-going service coordination that focuses on achieving desired outcomes of the individual.
  • Maintain case files/case notes.
  • Establish and maintain contact with providers to ensure that each provider is trained on the Individual Service Plan (ISP) and is aware of the services/supports expectations.
  • Complete cost projection worksheets and/or other required documents within the mandated/requested time frames necessary to authorize funding for services identified in the Individual Service Plan (ISP).
  • Take actions necessary to remediate any immediate concerns regarding the individuals health and welfare.
  • Work within a team environment with supportive supervisors!

Minimum Qualifications

  • Bachelors Degree.
  • Must successfully pass State and/or Federal Criminal Background Check*, ODODD Abuse Registry, Nurse Registry, Office of Inspector General, Sex Offender Registry, Offender Registry, and SAM Database required by law or rule.
  • Must successfully complete pre-employment physical including drug test.
  • Must obtain and maintain certification identified for the position by the Ohio Department of Developmental Disabilities (at hire).
  • Must maintain valid Ohio drivers license in accordance with Board policy.

*Greene County Developmental Disabilities is required pursuant to to Ohio Administrative Code Section, OAC 5123-2-02 to conduct background investigations prior to employment. It's further noted that per OAC 5123-2-02, there are five tiers of disqualifying offenses with corresponding time periods that preclude an applicant from being employed with this agency.
