Are you passionate about making a real difference in patients' lives? We are hiring a Case Manager to join our team in South Orange County. In this critical role, you’ll coordinate non-clinical care, ensure smooth care transitions, and help drive positive health outcomes for our patients.
Schedule: Full time , Contract (3-5 months)
Location: Lake Forest
About the Role:
As a Case Manager, you'll work closely with healthcare providers, referral specialists, and community resources to develop and manage individualized care plans that address both medical and non-medical needs. You'll utilize our electronic health record (EHR) system to track patient progress, document activities, and ensure adherence to care plans. Your expertise will help us ensure compliance with HEDIS and FQHC clinical performance measures, conducting follow-ups, preventive screenings, and patient education.
Key Responsibilities:
- Coordinate follow-up appointments, tests, and specialist referrals after hospital discharges to ensure continuity of care
- Engage patients with medical, mental health, and/or substance utilization disorders in health support services utilizing evidenced-based engagement skills
- Conduct holistic health assessments and ongoing evaluations to identify patient needs and develop targeted interventions
- Facilitate communication and coordination among the healthcare team to minimize fragmentation and maximize delivery of appropriate care
- Provide interventions focused on removing patient barriers to health improvement, promoting positive behavior change, and addressing caregiver needs
- Maintain accurate and timely documentation that supports effective and efficient management of patients

Requirements:
- High school/GED required
- Bilingual (English/Spanish)
- Two years’ experience working in a healthcare setting is required
- Experience working in case management within the medical office and/or behavioral health; ideally in environments with individuals who struggle with chronic illness, substance use disorders, and mental health issues
- Valid CA Driver’s License and proof of auto insurance required
- BLS certification required
Preferred Qualifications:
- Bachelor’s of Science in health or human services related field; other undergraduate degrees will be considered
- Licensed Vocational Nurse (LVN) with case management experience
- Certified Case Manager (CCM) certification
- Enhanced Care Management (ECM) experience
