Position Profile

We seek a compassionate, detail oriented, and outcomes driven Quality Coordinator to join our Quality team. The Clinical Quality Coordinator serves the Clinical Quality Team in completing important projects and tasks to contribute to the objectives of the Clinical Quality Program. The objectives include:

  • Satisfying quality measures
  • Closing gaps in patient care
  • Ensuring documentation accurately reflects the patients' condition(s)

Key Responsibilities

  • Performing chart reviews for documentation of quality care.
  • Reviewing reports, data, and medical records to identify opportunities to improve care quality and identify trends related to care quality.
  • Communicating with patients and clinical team members to achieve program objectives.
  • Participating in meetings and calls with internal constituents, payers and patients as needed.
  • Assisting with other duties as assigned.

REQUIRED QUALIFICATIONS:

  • High School graduate or equivalent education required.
  • Working knowledge of the patient record in the EMR (preferably eCW)
  • Medical knowledge associated with experience such as MA, LPN, biller/coder as relates to clinical quality including medical terminology, diagnoses, and medications.
  • Ability to effectively utilize technology to perform clinical quality tasks.
  • Prefer previous experience with clinical quality initiatives.
  • Knowledge of HEDIS and Medicare
  • Demonstrated ability to organize, coordinate, prioritize, and facilitate multiple tasks.
  • Excellent verbal and written communication skills.
  • Proficient computer skills including Excel, Word, Microsoft Office, Outlook and others
  • Professional demeanor and appearance.
